5. Requesting access to a record
A request for access should be made using the prescribed PAIA Form 2 and submitted to the Information Officer. The requester must provide sufficient information to identify the requested record and explain the applicable right where required.

6. Fees and decisions
Any fees, decision periods, grounds for refusal and complaint rights applicable to a PAIA request are determined in accordance with PAIA and its regulations.
